BMI pelvic floor treatment terms and conditions
The assessment price
1. Two appointments with a BMI Healthcare women’s health physiotherapist - £160
The physiotherapy price
2. Four sessions of physiotherapy, with a BMI Healthcare women’s physiotherapist - £260. Each session will last 45 minutes.
3. Both prices exclude:
3.1. Any diagnostic tests or investigations required
3.2. Surgery on other inpatient or outpatient treatments
3.3. Medication
3.4. Hospital accommodation
3.5. Meals
3.6. Transport
3.7. Sundry items (phone calls, visitor meals, newspapers)
Your appointment
4. The hospital reserves the right to refuse to make an appointment with a patient for any reason.
5. We aim to offer all patients their first choice of appointment, however we reserve the right to determine the date of appointment of the patient and if necessary change the date of appointment. Prior notice will be given, wherever possible, should the appointment date need to be changed.
6. Costs that are incurred that are not included in the assessment or physiotherapy price will be charged for at the hospital’s standard rates. Patients will be advised of any additional costs before they are incurred.
7. Both the two assessment appointments and four subsequent physiotherapy sessions, if appropriate, must be taken within a six-month timeframe.
8. If you need additional physiotherapy sessions, these can be reserved at the physiotherapy price for four sessions.
Each additional block of four sessions must be taken within a six-month timeframe.
Written quotation
9. You will receive a written quotation for your assessment and physiotherapy sessions in your appointment letter.
10. Unless otherwise agreed by us in writing, the price of the service will be the price set out in the quotation letter.
11. We reserve the right to withdraw the quoted price at any time prior to the assessment or physiotherapy.
12. The quoted price is valid for a period of 60 days from the date of issue and the first assessment appointment must be taken within this 60 day period.
Payment
13. For patients with health insurance (or equivalent) that provides reimbursement for this treatment, settlement arrangements are per the prevailing agreement between the hospital and insurer/third party.
14. For all other patients, payment for the two assessment appointments must be made in full either before or on the day of the first appointment.
15. No refund will be given if you fail to attend either of your assessment appointments.
16. Payment for any subsequent physiotherapy sessions (in blocks of four sessions) must be made in full before or on the day of the 1st physiotherapy session.
17. No refund will be given if you fail to attend any or all of the four physiotherapy sessions.
18. You may need a GP referral to validate your private medical insurance claim.
19. Payment may be made using most debit and credit cards or the BMI Card. Personal cheques are accepted by prior arrangement.
20. Any further treatment offered, including diagnostic tests, will incur extra charges and full payment will be required prior to the treatment being undertaken.
21. You will be asked to complete a patient registration form at your first appointment. Completion and signature of this form indicates your acceptance of these terms and conditions.
