Osteoporosis terms and conditions

The osteoporosis screening price

1. Includes a DEXA scan and consultant interpretation

The price excludes:

2.1 Any additional diagnostic tests or investigations required

2.2 Surgery or other inpatient or outpatient treatments

2.3 Medication

2.4 Hospital accommodation

2.5 Meals

2.6 Transport

2.7 Sundry items (phone calls, visitor meals, newspapers)

Your appointment

3. The hospital reserves the right to refuse to make an appointment with a patient for any reason.

4. We aim to offer all patients their first choice of appointment, however we reserve the right to determine the date of appointment of the patient and if necessary change the date of appointment. Prior notice will be given, wherever possible, should the appointment date need to be changed.

5. Costs that are incurred that are not included in the osteoporosis screening price will be charged for at the hospital’s standard rates. Patients will be advised of any additional costs before they are incurred.

Written quotation

6. You will receive a written quotation for your osteoporosis screen in your appointment letter.

7. Unless otherwise agreed by us in writing, the price of the service will be the price set out in the quotation letter.

8. We reserve the right to withdraw the quoted price at any time prior to the screen.

9. The quoted price is valid for a period of 60 days from the date of issue and the screening appointment must be taken within this 60 day period.

Payment

10. For patients with health insurance (or equivalent) that provides reimbursement for this service, settlement arrangements are per the prevailing agreement between the hospital and insurer/third party.

11. For all other patients, payment for the osteoporosis screening must be made in full either before or on the day of the first appointment.

12. No refund will be given if you fail to attend your screening appointment.

13. You may need a GP referral to validate your private medical insurance claim.

14. Payment may be made using most debit and credit cards or the BMI Card. Personal cheques are accepted by prior arrangement.

15. Any further treatment offered, including diagnostic tests, will incur extra charges and full payment will be required prior to the treatment being undertaken.

16. You will be asked to complete a patient registration form at your first appointment. Completion and signature of this form indicates your acceptance of these terms and conditions.
